It looks like something from a lucid nightmare. The way the mouth almost seems to melt together, the protrusions down his back seem like shards of his spine jutting outward, the thorny vines give a feel of claustrophobia and the background looks like either the approach of a black hole to which there is no escape, and provides a since of dread and inevitability or it is a very large demonic eye, which is constantly watching and again there is no escape. All in all pretty cool picture!
